Is it available somewhere 5S electronic form where the data will automatically enroll on the SQL server from multiple locations?

I guess I don't understand what you are searching for.  Are you looking for a system that automatically tracks 5S data or a system to capture 5S audit data?

In my experience 5S data can only be collected by going to the Gemba and seeing with your own eyes.  I find it best to print out a version of my audit document and physically walk through every part of the facility.  (However, with the wide spread use of tablet computers, this manual process is under the microscope for improvement.)

As far as electronic documentation, a simple excel sheet can be created to auto-populate metrics, charts, and graphs, based on the current and historical audit data.  Sharing of this data can be done through processes such as a corporate file server or through cloud based solutions like google docs or dropbox. 

I think it is important to focus on the Lean principles that insist on using the simplest effective process possible.

I agree with Wesley. Many companies have made their own electronic 5S audit forms which populate various charts and data. Not sure if that's what you're looking for.

I think the goal is to have a form that is scanned and the data of the selected filled in fields (such as check-mark or numbers) to be gathered and processed to a database for calculations of frequency/pareto.

If that is the the intent Piotr, yes, this is possible.  I would caution the use of such however. Do a cost analysis first to see the ROI.  Such processes are often reserved for mass information gathering, like that of education scores and census.  
Start with a search of, "auto data entry of forms", "auto data systems"

Consider the alternate...  make the data viable at the moment and implement PDCA immediately. With this method I do a summary at the end of each month to a pareto (for trend views to management and workers)... it takes me all of 15 min a month. 

As I understood by my experience is Mr .Piotr Henke wan to save time in doing 5S audit by electronics device to reduce repetitive clerical work like first auditing gemba on paper then transferring this hard-copy data into excel or any template soft copy,making action plan & circulating PDF to each respective person or department .

I can suggest one thing is for this you can use IAudit Application in android mobile ,by using this you can do Audit on gemba & no need to fill separately on paper immediately we can take photo for each abnormal points.

This will be useful ,by using this you can save time.

As for the general analysis of data from the whole plant. Automating data collection speeds up data analysis and allows for quicker response.

Quickly we have information on what happens at the various departments, what problems are there and what scale.

I hope my idea is understandable.

Piotr henke you are very much true ,quicker response is the very much necessary if you are performing work of coordinator.
